Contract Updates Effective September 1st, 2024:
– The Academic Advising Form (AAF) is only required during your first contract (no longer required for renewal contracts), unless you start an entirely new academic program, and a new contract is initiated.
– CCA of WA is no longer providing a completion incentive bonus, and it is no longer required to be employed at the same childcare facility for 6 months after scholarship completion.
– CCA of WA is now providing a stipend for books and materials that will be generated with the submission of your schedule and tuition & fees statement. Scholars will no longer need to submit reimbursement forms. The stipends will be $125/quarter OR $250/semester, up to $500 per contract year.

Pierce County Human Services (PCHS) Forms
The following forms are provided to assist you in obtaining your scholarship benefits.
PCHS Employment Release Time Agreement
This agreement is for those PCHS scholars who are employed at a licensed child care facility who would like to access the optional Release Time Benefit. This form must be submitted well in advance before the Release Time Claim form (please see deadlines on the form itself) and be signed by an authorized facility representative.

PCHS Academic Support Invoice Template
To be submitted with the Reimbursement Claim Form for the PCHS scholarship. This invoice template is designed to provide documentation of academic support where receipts are not normally available.
